Contract between Town Well Being ltd and Participants of the Group Therapy Program Roots to Wings
This contract (hereinafter referred to as the "Contract") is entered into between Town Well Being ltd (hereinafter referred to as the "Provider") and the participant of the "Roots to Wings" group therapy program (hereinafter referred to as the "Participant").
1. Agreement to Participate:
By signing this Contract, the Participant agrees to participate in the "Roots to Wings" group therapy program provided by the Provider. The Participant acknowledges that they have read, understood, and agreed to comply with the terms and conditions set forth herein.
2. Program Fee Structure:
The Participant agrees to pay the program fees according to one of the following options:
a. Option 1: A total payment of £5000, submitted in full at the time of signing this Contract. A deposit of £500 is required upon signing this Contract, with the remaining balance of £4500 due prior to the start of the program.
b. Option 2: A total payment of £6500, divided into three instalments. A deposit of £500 is required upon signing this Contract. The remaining balance of £6000 shall be paid in three equal instalments of £2000 each. The first instalment is due prior to the start of the program, and subsequent instalments are due at monthly intervals.
3. Program Structure and Meeting Schedule:
a. The "Roots to Wings" group therapy program consists of one weekend per month for group meetings and two one-to-one meetings per month.
b. The specific dates for group meetings and one-to-one meetings will be agreed upon between the Participant and the Provider prior to the start of the program.
c. The Participant understands that attending one-to-one meetings is a prerequisite for attending the subsequent group meeting. Therefore, the Participant must complete their one-to-one meeting before participating in the next scheduled group meeting.
4. Late Arrival:
The Participant is expected to arrive on time for all scheduled group therapy sessions. In the event of late arrival, it is the Participant's responsibility to inform the Provider in advance. The Provider reserves the right to modify or reschedule sessions due to late arrival, at their discretion.
5. Phones and Electronic Devices:
The Participant agrees that the use of phones and other electronic devices during therapy sessions is strictly prohibited. All electronic devices must be turned off or silenced and stored away for the duration of the session. This policy aims to create a distraction-free environment, promote active engagement, and ensure the privacy and confidentiality of all participants.
6. Confidentiality:
The Participant acknowledges and agrees to maintain the utmost confidentiality regarding any personal information or experiences shared by other participants during the course of the program. Violations of confidentiality may result in termination from the program without refund.
7. Governing Law and Jurisdiction:
This Contract shall be governed by and construed in accordance with the laws of [Jurisdiction]. Any disputes arising under or in connection with this Contract shall be subject to the exclusive jurisdiction of the courts of [Jurisdiction].
8. Attendance and Participation:
The Participant agrees to attend all scheduled group therapy sessions to the best of their abilities. Regular attendance is essential for successful progress and growth within the program. The Participant also acknowledges the importance of active participation during sessions, including sharing experiences, providing feedback, and respecting the thoughts and emotions of other participants.
9. Code of Conduct:
The Participant agrees to adhere to a code of conduct that fosters a safe and respectful environment for all participants. This includes treating others with dignity and respect, refraining from engaging in harmful behaviours, maintaining confidentiality, and complying with any additional rules or guidelines set forth by the Provider.
10. Termination or Suspension:
The Provider reserves the right to terminate or suspend the Participant's involvement in the program in cases where the Participant violates the code of conduct, displays disruptive behaviour, or fails to comply with the terms and conditions outlined in this Contract. In such instances, no refunds will be provided, and any outstanding fees will remain payable.
11. Boundaries and Informed Consent:
The Participant understands that group therapy encourages openness, vulnerability, and exploration of personal experiences. However, it is crucial to respect and maintain appropriate boundaries. The Participant has been informed and understands that they have the right to decline participation or disclosure of any personal information during sessions without facing negative consequences.
12. Non-Medical Advice:
The Participant acknowledges that the group therapy program offered by the Provider is not intended to replace professional medical or psychiatric advice and treatment. The Participant is encouraged to seek appropriate medical or psychological help in addition to participating in the program if needed.
13. Liability Waiver:
The Participant understands and acknowledges that participation in the group therapy program carries inherent risks and that the Provider, and its staff shall not be held liable for any physical, emotional, or psychological injuries or damages that may occur during the program.
14. Modification or Termination of the Program:
The Provider reserves the right to modify, suspend, or terminate the group therapy program at any time. In such cases, the Provider will provide reasonable notice to the Participant and refund any applicable fees for sessions not yet attended.
15. Data Recording for Quality Assurance and Supervision:
The Participant acknowledges and agrees that the Provider may record, store, and use data collected during the group therapy program for quality assurance and supervision purposes. This may include audio or video recording of therapy sessions, written notes, and other related materials.
a. Confidentiality of Recorded Data:
The Provider is committed to maintaining the confidentiality and privacy of all recorded data. Any recorded materials will be stored securely and accessed only by authorized individuals involved in quality assurance and supervision processes. All recorded data will be de-identified to ensure the anonymity of participants.
b. Purpose of Data Recording:
The recording of therapy sessions and related data is intended to support the ongoing professional development of therapists and ensure the delivery of high-quality services. It may be used for supervision, consultation, training, research, program evaluation, and adherence to ethical and professional standards.
c. Consent for Data Recording:
By signing this Contract, the Participant grants their informed consent for the Provider to record, store, and use data collected during the group therapy program for the purposes described above. The Participant understands that their identity will remain confidential and that the recorded data will be used solely for professional purposes.
d. Limitations on Data Use:
The recorded data will not be shared or used in any way that could personally identify the Participant without obtaining explicit written consent. The Provider will ensure compliance with all applicable laws and regulations governing data protection and confidentiality.
e. Participant's Right to Withdraw Consent:
The Participant has the right to withdraw their consent for the recording and use of their data at any time during the program. To withdraw consent, the Participant must provide written notice to the Provider. However, the Participant acknowledges that withdrawn consent does not affect any recordings made prior to the date of withdrawal.
16. Cancellation and Refunds:
a. The Participant acknowledges that the deposit amount is non-refundable in all circumstances.
b. Once the program has commenced, the Participant is not liable to withdraw or request a refund of any portion of the program fees.
c. In the event that the Participant is unable to attend specific sessions due to force majeure reasons such as serious illness, natural disasters, or other unforeseen circumstances beyond their control, the Provider will make reasonable efforts to offer an alternative session or arrangement for the Participant to catch up on missed content.
d. The Participant understands that any catch-up sessions or alternative arrangements offered by the Provider are contingent upon availability and may not be equivalent to the originally scheduled sessions.
